The Representative David B. Sullivan NAMI Golf Tournament

Gather your friends, family, and co-workers and join us for a fun day of golf while participating in The Representative David B. Sullivan “Take a Shot Against Stigma” NAMI Golf Tournament to be held 1:00pm Sunday, August 15, 2010 at the Swansea Country Club, 299 Market Street, Swansea MA. Proceeds benefit NAMI of Greater Fall River– serving the Fall River, Taunton, New Bedford, and nearby Rhode Island areas. Registration begins at 11:00am.

NAMI is the National Alliance on Mental Illness with local affiliates based throughout the country. NAMI Greater Fall River is a grassroots non-profit 501(c) (3) NAMI affiliate dedicated to the eradication of mental illness and to improving the quality of life for persons of all ages who are affected by mental illness. NAMI Greater Fall River’s purpose is to help families in crisis through mutual aid and support; to advocate for better treatment and needed services; and to educate the public about mental illness and the needs of the mentally ill in order to help combat the stigma associated with mental illness. Our aim is to help persons with mental illness achieve a sense of dignity with the highest degree of independence and productivity.

WHY GOLF? Because one in four Americans will have a diagnosable mental illness during their lifetime; and mental illness causes more disability than any other class of illness in the nation.

Golf With Us to raise funds to educate the general public about mental illness.

“Putt out” stigma and the negative stereotypes associated with mental illness.

“Chip In” to promote the fact that recovery is real; people with mental illness are integrated and contributing members of their communities leading productive lives.

“Drive” to empower people with mental health disorders to advocate for education, employment, and affordable housing.

“Pitch In” because mental illness does not discriminate. No one is immune.

The fight against mental illness is waged by countless heroes. From the parent fighting to reclaim his/her son or daughter; to the social worker looking for clients lost to the streets; to the policeman who knows when someone needs medical help and not prison time. These are the people who make a difference!!! By helping to make one life better; one family stronger; and one burden lighter you can also make a difference and become one of our “heroes in the fight”.

We are seeking event sponsors at all levels for our golf tournament. We are asking you to help by participating as a player, becoming a sponsor, or donating gifts or services. NAMI of Greater Fall River is a non-profit 501(c) (3) organization and ALL DONATIONS ARE TAX DEDUCTIBLE to the full extent allowed by law. “Tee it up” for a day of friendly golf competition while supporting our charitable event. The golf will be fun, the company will be better…but the cause is why you MUST be there!!! With your generosity and support this event will become a huge success for years to come!!!

ENTRY FEE is $95.00 per golfer which will include greens fee, driving cart, range balls, skill contests (closest to the pin, longest and straightest drives), post golf dinner, and awards. Optional extras per golfer include: mulligans $5.00 each or 3 for $10.00 (max); a $5.00 split the pot hole on a par 3; a $5.00 drop area on a par 5 counting as your drive; a $5.00 50/50 putting contest, a $5.00 poker hand, a huge raffle, and more.

GOLD CORPORATE SPONSORSHIPS are available for $750.00 and includes one (1) complimentary foursome (4 golfers), four (4) complimentary dinner only guests, one (1) tee/green sponsorship with signage, your company banner prominently displayed on the course for all to see (you provide the banner), prominent mention in any press releases and promo materials, your company logo displayed on our website, and recognition at our awards dinner following golf.

SILVER COMPANY BANNER SPONSORSHIPS are available for $375.00 and includes a complimentary twosome (2 golfers), two (2) dinner only guests, one tee/green sign, a small company banner prominently displayed (you supply the banner), and recognition on our web site and at the awards ceremony following golf.

TEE/GREEN SIGN SPONSORSHIPS are available for $125.00 for one (1) tee/green sign. A three (3) tee/green sign sponsorship is available for $225.00. A five (5) tee/green sign sponsorship is available for $300.00. This would be a great opportunity to advertise for your company or organization on one of the tee/green signs that will be prominently located alongside the tees and greens throughout the golf course.

LUNCH AND AWARDS SPONSORSHIPS are available for $350.00 each. Help provide the golfers with a sandwich after nine holes or sponsor any of the prizes and awards to be given out. Signage and recognition included.

REFRESHMENT CART SPONSORSHIPS are available for $250.00. Help the golfers remain cool, calm, and collected out on the course. Signage and recognition included.

MONETARY DONATIONS, GIFT CERTIFICATES, and PRIZE DONATIONS are always welcome. Got items to donate? Don’t play golf? No problem!!! Join us for the event dinner only by purchasing “dinner only” tickets for $30.00. This will be a fun filled social event and a memorable night for all. Enjoy the night with golfers, families, friends, and special guests.

Your support and financial help is critical in these tough times. We are a strictly volunteer affiliate and 100% of any money raised will go directly to fund our various programs and be used to support persons affected by mental illness and their families.

Once registration is paid there will be no refunds for no-shows.

Proper Golf attire. Collared shirts are required. No cutoffs.

Anyone bringing alcohol from off premises will be asked to leave without refund.

Swansea Country Club is green friendly—no spikes allowed.

Rescheduling will be permitted only if Swansea Country Club management officially closes the course. If no alternate date is available, the greens and cart fees will be pro-rated. Once the tournament has completed three holes (or up to nine holes) rain checks will be issued for the nine holes. Once a tournament has completed twelve holes the tournament is considered complete and no rain check will be issued. The food and beverage function CANNOT be rescheduled or refunded due to the advance ordering of perishable food.

Neither NAMI of Greater Fall River nor the tournament is responsible for damage to golf carts caused by participating golfers or for any/all other property damage or injuries to oneself or others resulting from playing in the tournament. Responsible golfers will be held accountable.
